Developmental Disabilities Services

    Recreation and Respite Programs

    Medicaid Service Coordination: An individualized service of advocacy, referral, support and information to the developmentally disabled. The coordinator monitors and manages services for quality and appropriateness for the consumer's development.

    Leisure Time Recreation Program: A recreation and socialization program for WillowbrookClass Clients. Program meet Tuesday and Thursday evenings 4:30pm - 9:00pm and Saturdays from 10:00 am - 4:00pm. Consumers have a choice of games, gym, arts & crafts activities in which to engage. Core services include meals and transportation.

    Leisure Time II & Leisurely Yours Programs: Family support recreation/respite programs for consumers who reside in the community with family members. Program operates Saturdays and Sunday, 10:00am - 4:00pm. Sewing, cooking, music and dance are the major activities. Transportation is available.

    Leisurely Yours After School Programs: A homework assistance and recreation program for 12 to 21 year old school enrolled consumers. The program follows the New York City School calendar, September through to June. Transportation from the program site to case address and meals, are the core service.

    Project Career Evening Program: A homework assistance and recreation program for 12 to 21 year old school enrolled consumers. The program follows the New York City School calendar, September through to June. Transportation from the program site to case address and meals, are the core service.

    In Home Residential Services: A program that provides in home habilitation services where consumers work on personal goals and community intergration. Habilitation plans are individualized to address specific needs for enhancement and development of skills.

    Day Programs and Service

    Day Habilitation Services: A Five day week program designed to assit ins participants to live intergrated and productive lives through communty service, participation in community events and the cultural life of the community.

    The Best of Me Community Habilitation Services: A Five day week program designed to assit ins participants to live intergrated and productive lives through communty service, participation in community events and the cultural life of the community.

    Sports Program: A seasonal program serving the developmentally disabled adults. This program helps to improve consumer's motor skills, socialization and community inclusion, and runs from September through June.
